About 360 Living

What should I expect from 360 Living?
We provide high quality care that focuses on prevention and education/treatment of chronic health conditions. This will be accomplished more easily as we will extend our office visits since we are not constrained to the 15-minute model. This extra time will allow for deeper investigation into causes of health-related concerns and patient education.
What does 360 Living provide that I can’t get at other physicians’ offices?
Most of our patients have chosen us because of our ability and knowledge to be proactive, utilizing many different modalities compared to a traditional medical practitioner. These modalities include diet and nutrition, and in-depth metabolic screening to achieve total wellness. Unfortunately, many doctors tend to focus on treating an isolated symptom without looking at the entire history and environmental factors that contribute to illness. At 360 Living, we embrace continuous learning and staying up-to-date on new advances in functional medicine.
About the Charges for Our Practitionersʼ Services
Some patients may have the mistaken impression that our practitioners take home the majority of the fees we charge for their services, and that the practitioners have a great deal of leeway to offer discounts for those fees. In fact, our practitioners take home only a fraction of the fees collected for services. This is because a clinic like ours requires trained staff and professional continuing education. The majority of our fees support the overall mission of providing high quality health care, not the practitionersʼ paychecks. Our practitioners have chosen this work because it is their passion and their calling, and certainly not because it is a way to make a substantial income. In fact, most medical doctors who choose to practice functional medicine know that their income will be significantly lower than it would be if they were practicing in a more conventional manner that is fully supported by the health care reimbursement system.
Why We Sell Nutritional Supplements and How We Price Them
We recommend nutritional supplements as an adjunct to dietary and lifestyle modification. This approach is central to the well-researched, science-based practice of functional medicine. We sell therapeutic quality nutritional supplements as a service to our patients. With a few exceptions, we do not sell nutritional products of similar quality to those that are widely available over the counter. We purchase high-quality nutritional products from the top nutritional companies in North America.

Scheduling & Visits

What to Expect at Your 360 Living Visit?
At 360 Living your initial visit must be in person. Subsequent visits may be scheduled with Dr. Keith or a member of her care team by phone or video conference.
Email Etiquette
You have unlimited email contact with your 360 Living care team. However, if an email question or request is deemed by one of our staff to be too complicated or involved for a response via email we may, at our discretion, require a scheduled a phone, video conference or in person visit instead of responding via email. Please allow 24-48 hours for response to an e-mail. Weekend and holiday e-mail may not be addressed until the next business day.
Scheduling & Cancellations
There is a 24-hour cancellation policy for all appointments with any an 360 Living care team member. Canceling within 24-hours will result in a $50 charge to the credit card on file.

No-shows:If you do not show up to a phone, video/telemedicine, or in person appointment, and fail to notify the specific person you have an appointment with either via email or by changing the appointment using the online scheduling tool, you will be charged $100 to the card on file.

Financial Policy, Insurance & Billing

An Explanation of Our Financial Policy
At 360 Living, we are passionate about what we do, and we feel that we have a calling to provide the highest quality functional health care to as many people as possible. Just as our services are unique to this region, our financial policies set us apart from mainstream medicine. Please read this Policy to answer questions you or your family members may have about the rationale for our financial practices. If, after reading this, you still have questions, feel free to speak to one of our helpful Care Team members.
We Do Not Bill Insurance Companies
The cost of any services provided by 360 Living, which are not included in the initial contracted fees, will be billed to you directly.

 

Non-360 Living expenses such as lab tests may be covered by your health insurance plan per that plan’s rules.

360 Living will not submit a claim to insurance on your behalf. We cannot assist you with claim resolution for our services.

Why 360 Living Does Not Accept Insurance
Some of our patients have asked us why we do not bill insurance directly. We have chosen not to bill insurance directly for the following reasons:

 

When clinics bill insurance companies directly, the doctors are required to become participating providers. The doctors must sign a contract that allows the insurance company to determine which services they will and will not provide, and how much they can charge for those services. In general, insurance companies are not focused on any preventative or wellness services. They are heavily invested in the conventional model of health care that too often relies on drugs and surgery. We are committed to the functional medicine model that addresses the underlying causes of your symptoms with specific nutritional and lifestyle recommendations.

A participating provider must agree to accept the fees the insurance company establishes, regardless of whether the fees are reasonable or applicable to that practice. In general, these established fees cover the actual cost of the briefest (and we believe the lowest quality) care. Doctors who are participating providers are required to accept discounted fees for their services. Therefore, the clinic must write off the difference, often as much as fifty percent or more of the doctorʼs fee for service. Most doctors and clinics cope with the requirements of being participating providers by keeping their office visits very brief, so that they can see many patients within a given timeframe. We believe you deserve more time with the physician and Care Team.

Ironically, some of our patients complain about their extremely brief office visits in other medical practices, while at the same time expressing frustration that we do not accept insurance. Unfortunately, we have found that we cannot stay in the insurance networks and provide the time-intensive, well-researched, expert care that you will get at 360 Living.

We Are Not A Conventional Primary Care Physician
At 360 Living, our desire is not to take the place of your primary care physician. We desire to partner with you and your primary care physician and/or other specialty physicians assisting with your care management. We are a team with the ultimate goal of seeing you succeed on your wellness journey

It is important to note the following however:

1. We do not accept or bill insurance.

2. We do not fulfill an insurance company’s requirement for an in-network primary care physician as we are not in-network with any insurance plan.

3. We are not available for same-day emergency appointments.

4. We do not have a 24-hour answering service and do not take calls on weekends or holidays. If you need to be seen urgently or have a medical emergency you would need to visit an ER or urgent care center.

About Lab Tests
  • Most lab tests ordered by a 360 Living care team member will be covered in your initial contracted fees.
  • The specific tests ordered will be personalized to you based on your initial evaluation with a 360 Living care team member and may include blood, saliva, urine, or stool testing.
  • We will make every effort to make you fully aware of any additional lab fees that may be incurred, and to give you the opportunity to choose which lab tests you feel comfortable paying for.
  • You are under no obligation to purchase any specialty testing.
  • Should any lab results indicate you may need further testing or treatment beyond the scope of 360 Living, you will be directed to continue your care with your primary care physician or a relevant specialist.
